Welcome, Well, and Whole Peer Support Program

Clear Path for Veterans (CPV) provides supportive services and collaborates with a network of local businesses and nonprofits to connect service members and veterans in a 33-county service area across Upstate New York to the resources they need. CPV’s Peer Support Program reaches veterans and active-duty service members exactly where they are, providing close, individualized support for those who need assistance in pursuing their personal goals and objectives. Peer Support Managers are veterans themselves, enabling them to build trust and mitigate their clients’ social isolation as they work with veterans to improve their long-term stability and wellbeing. This NFL-BWF Healthy Lifestyles and Creating Community grant will increase the capacity of CPV’s Peer Support Program to reach and serve veterans living in Jefferson, Lewis, and Wayne counties. CPV will work to close resource and service gaps in these underserved, rural communities through targeted outreach and collaboration with community partners. This grant will ensure that at least 75 veteran and military households living in underserved counties access peer support, food assistance, and resource navigation to reduce isolation and improve their quality of life.
